To ensure timely entry at Department please note:
Membership Applications are to be filled out by the new member (or returning member over 2 years).
Please verify new member and adjutant have each signed the form and the single eligibility person on the form has legion member # unless eligibility person is deceased. Continue to report served era and relationship for all forms.
Transmittal forms accompany membership applications, should be turned in weekly or when the unit has 5 members. The members should be listed by category alphabetically.
Please do not forget to include checks.
Member Data form is used for transfers (signatures please), address changes, name changes, update to information (i.e. phone, email, address) member discontinue and the death of a member (please include obituary).
