up-to-date information for your Unit.
Please submit this form with payment to Department - cost is just $10 per person
(limit 2 persons per unit).

Each Unit can have access to the National Membership Database, ALAMIS for payments for renewals and new memberships. After approval, you will not need to send in any more transmittals to Department. You will enter all the information and payment yourself.
